This document outlines the University's accountable plan procedures to assure timely reporting of travel expenses.

This worksheet can be used to determine the reimbursement amount allowed for individuals electing to drive instead of fly on their trip.

This form is needed for trips longer than 60 days but not more than a year. The form needs to be filled out every 90 days.

This affidavit needs to be filled out when a receipt of greater than $50 was lost or otherwise unobtainable. The form should be attached to the expense report in lieu of the lost receipt.

This federal form needs to be filled out by any traveler traveling into or out of the United States and is carrying $10,000 or more in monetary instruments.

The university's policy regarding undergraduate student travel has a provision for exceptions. To obtain approval for an exception, a form must be competed and signed by the Dean/Director and the Managing Director of Risk Management & Safety. Please see the highlighted sections below.
